TERMS OF AGREEMENT
I am enrolling in the educational and consulting services of Nutritional Solutions so I can learn about health factors within my control: my diet, nutrition, and lifestyle choices. By optimizing these factors, I believe I can nourish my health and well-being. I understand that the staff at Nutritional Solutions and Jeanne M. Wallace, PhD, CNC, are not medical physicians. They do not dispense medical advice nor prescribe treatment. I understand these services are not a substitute for medical care and are not intended to diagnose, treat, alleviate, or care for disease. Rather they are intended to complement my medical care. Methods of nutritional evaluation or testing made available to me are not intended to diagnose disease. Rather, these assessments serve as a guide to help me develop an appropriate nutrition program tailored to my individual needs and monitor my progress in achieving my wellness goals.
I understand that any personal information I supply to Nutritional Solutions will be kept strictly confidential unless I consent to share it. In the forums and discussion areas of this program, I agree to maintain strict confidentiality regarding other participants with whom I may interact. In addition, I agree to abide by the Rules of Conduct outlined below and will refrain from posting:
- Random or irrelevant content, inappropriate, offensive or violent language
- Hateful or discriminatory comments regarding race, ethnicity, religion, gender, disability, sexual orientation or political beliefs
- Attacks on specific groups or any comments meant to harass, threaten or abuse an individual
- Links or comments containing sexually explicit content
- Commercial solicitations or the promotion of a competitor
- Spam, link baiting or files containing viruses that could damage the operation of other people’s computers or mobile devices
Furthermore, I agree that all of the information I receive from Nutritional Solutions—videos, presentations, PDF reports, handouts, worksheets, and such—is for the sole use of me and immediate family members who may be assisting me with my care. No part of this information may be reproduced, stored in or introduced into a retrieval system, or transmitted, in any form or by any means (electronic, mechanical, photocopying, recording or otherwise) without the prior written permission of Nutritional Solutions. I agree that I will not participate in nor encourage electronic piracy of Nutritional Solutions' copyrightable materials.
The Materials on this Site are copyrighted, and any unauthorized use of any Materials on this Site may violate copyright, trademark, and other laws. Nutritional Solutions authorizes each User to view and download one copy of the Materials and a maximum of TWO COPIES of the Materials may be printed provided that Users make no modifications to the Materials. Dr. Wallace retains all copyright and other proprietary notices contained in the original Materials on any copies of the Materials. Users may not modify the Materials at this Site in any way or reproduce, share or distribute them. Users will keep all Materials confidential, and will not sell, auction, loan, rent, give away, describe, summarize, or otherwise reveal the Materials or their contents, to any other person or entity. Any breach of these Terms of Use automatically terminates your authorized use of the Site.
This agreement is a release of potential liability. I agree to hold Nutritional Solutions and its employees harmless for claims or damages in connection with our work together. In addition, I agree to abide by the guidelines set forth for community and social interaction on forums.
